然而，這只是二分之一的她。積極、強大、陽光……她選擇性地把這樣燦爛的自己展示給觀眾。

看到李玟去世的消息時，我的第一反應是不可置信，而當看到她自殺時，更是震驚不已，原來她飽受抑郁癥折磨已經(jīng)很久很久。抑郁癥，英文是 depressive disorder，或簡稱為 depression，許多患者需要服用多用抗抑郁藥才能緩解。我選了一篇《經(jīng)濟學人》刊登的文章，和各位一起閱讀。內(nèi)容較長，生詞較多，大家慢慢看。

詞根詞綴

這個詞由 anti- 前綴和 depressant 構成。anit- 可以表示「抗……的」，比如，antibiotic 是「抗生素」。depressant 又可以進一步拆分為 depress「使抑郁」 和 -ant，后者表示「用于做某事的東西」，所以 depressant 意思是「鎮(zhèn)靜劑；抑制藥」。到這兒整個單詞的意思就非常明朗了， antidepressant 指的是「抗抑郁藥」。

wean sb off sth

wean 本義是「給嬰兒或幼崽斷奶」。在漢語中，「斷奶」除了可以指不在給小寶貝們喂奶之外，還可以指戒除某種習慣、癮等。英語中的 wean 亦然。在演變過程中，它也常用來表示 to make sb gradually stop doing or using sth「使逐漸戒除惡習或依賴」。詞組中的 off 也可以用 from 替代。今年初，《經(jīng)濟學人》曾在文章中用到這一短語：

Duplicating green supply chains will make it costlier for America and the world to wean themselves off carbon.

Almost 35 years ago American drug regulators approved Prozac, the first in a series of blockbuster antidepressants known as selective serotonin re-uptake inhibitors (SSRIs). Prozac and its cousins were lauded by patients and doctors as miracle drugs. They lifted low moods quickly and seemed to have no drawbacks. Divorce, bereavement, problems at work—a daily pill was there to help with that, and anything else which made you sad. Many people have stayed on these drugs for life. In Western countries today between one person in seven and one in ten takes antidepressants.

Prozac

如果說有什么產(chǎn)品的譯名令人印象深刻，Prozac 的中文名「百憂解」絕對名列前茅。對飽受抑郁癥折磨的患者來說，世界被絕望、無助、黑暗充斥，而「百憂解」仿佛真的如其名所示，解除他們心中的百種憂愁。但這種藥在緩解抑郁癥的同時也會帶來副作用，比如，睡眠不安、食欲減退、口干、皮疹、怪夢、躁狂等。

blockbuster

熟詞生義

這個名詞在影視領域極為常用，因為它常用來指轟動一時的「大片」，但 blockbuster 還可以表示其他非常成功的事物，比如， a blockbuster novel「風靡一時的小說」，或者用來修飾其他名詞。所以文中 blockbuster antidepressants 是指那些效果顯著，非常成功的「抗抑郁藥」，我們也可以譯為「重磅抗抑郁藥」。

inhibitor

六級詞匯 + 考研詞匯

這是一個術語，指「抑制劑」。派生自動詞 inhibit，表示 to prevent something from growing or developing well「阻礙，抑制」。比如，inhibit brain development 是指「抑制大腦發(fā)育」。另外，它還常構成短語 inhibit sb from doing sth，用來表示「禁止，阻止」，可以替代 prevent sb from doing sth。

It could inhibit the poor from getting the medical care they need.

laud

laud 是動詞，表示 to praise someone or something「稱贊，贊美」，是一個比較正式的詞匯，所以官方文章經(jīng)常用到，不過口語使用頻率就比較低了。如果想表示「因某事稱贊某人」，我們會說 laud sb for sth。

bereavement

派生自動詞 bereave，表示「喪失」親友等。形容詞是 bereaved，大家可能在一些新聞、訃告中見到過。一方面，bereaved 可表示「喪失親友的」，比如 bereaved families 指的是「痛失親人的家庭」；另一方面，它和 the 搭配表示一類人， the bereaved 指的是「喪失親友的人們」，也就是「死者親友」。所以名詞 bereavement 用來指「喪親之痛」。

The shine of SSRIs has worn off. A growing number of studies show that they are less effective than thought. Drug companies often publish the results of clinical trials selectively, withholding those in which the drugs turn out not to work well. When the results of all trials submitted to America’s medicines regulator between 1979 and 2016 were scrutinised by independent scientists, it turned out that antidepressants had a substantial benefit beyond a placebo effect in only 15% of patients.

withhold 的英文釋義只有一條：to refuse to give someone something，但因為適用場景比較多，對應的中文釋義卻有好幾條。它可以是「拒絕給予」某物，也可以是「隱瞞，保留」證據(jù)、信息等。比如，withheld the evidence「隱瞞證據(jù)」，相當于 keep/hold sth back。在管理中，人們發(fā)現(xiàn)職權越大的人越容易指責別人。《經(jīng)濟學人》針對這一現(xiàn)象在 Bartleby 專欄寫道：

The person in the supervisor role was much more likely to agree that the transcriber was to blame for the errors and to want to withhold payment. Power and punitiveness went together.

在扮演主管?色的參與者中，認同轉(zhuǎn)錄員應為錯誤負責、并拒絕支付其報酬的人員比例更高。權力和苛責相伴而生。

scrutinise

六級詞匯

美式寫法是 scrutinize，表示 to examine someone or something very carefully「仔細檢查，細致審查」。名詞是 scrutiny。來看 2021 年 12 月六級閱讀的一個句子。

Social media screening is essentially scrutinising a job candidate's private life.

社交媒體篩選本質(zhì)上是在審查求職者的私生活。

placebo effect /pl??si?b??/

placebo 是「安慰劑」，所以 placebo effect 是「安慰劑效應」。這種效應于1955年由畢闕博士（Henry K. Beecher）提出。雖然醫(yī)生提供的是無效療法，但因為病人相信這些手段或者藥物有效，竟然改善了健康。由此說明精神力量在治療中有很大作用。

Several things are needed for change to happen. Doctors need guidelines on how to de-prescribe the drugs. Health-care insurers and providers such as Britain’s various national health services should start paying for ways of delivering the drugs that help those who wish to stop taking them but who need to taper off them in order to avoid severe withdrawal effects. These include liquid formulations, tapering strips which contain pills with progressively smaller drug concentrations, and the services of compounding pharmacies, which prepare bespoke doses. In the Netherlands, 70% of people using tapering strips have managed to quit successfully.

de-prescribe

四級詞匯 + 考研詞匯

很少有詞典收錄 de-prescribe，因為這是一個新詞。不過后半部分 prescribe 卻不是新人，意思是「給某人開處方，讓某人采用某療法」，那 de- 前綴表示什么意思呢？de- 有時相當于 reduce，表示「減少」。比如，devalue 是「使貶值」。在這里，de- 也是同樣的用法，所以 de-prescribe 義為「精簡處方」。這一舉措的目的在于減少患者的用藥負擔，因為有些藥品對患者的益處很小，甚至可能有損身體。

health-care insurers

insurer 通常指的是 a company that provides insurance「保險公司」而非真人，所以 health-care insurers 是指「醫(yī)療保險公司」。

taper off

taper 是「細蠟燭」，蠟燭越燒越短，所以，taper 還有個引申義，表示「逐漸縮小」。所以 taper off 這個詞組用來表示 to decrease gradually「逐漸變少，逐漸減弱」。

They are gradually tapering off production of the older models.

他們在逐步減少生產(chǎn)舊型號的產(chǎn)品。

withdrawal effects

四級詞匯

除了表示「提款，取款」外，withdrawal 還可以表示「脫癮，戒斷」，比如 drug withdrawal「藥物戒斷」。withdrawal effects 則是指藥物戒斷后出現(xiàn)的一些身份反應，即「戒斷反應」。

liquid formulations

四級詞匯

formulation 可以是化妝品、藥品的「配方，制劑」，也可以是制造而成的「配方產(chǎn)品」。文中 liquid formulations 指的是「液體制劑」。

progressively

考研詞匯

表示「逐漸地」，我們可能想到了 gradually，如果想讓措辭更加有特色，可以換成 progressively，因為后者也用來表示 steadily and continuously「持續(xù)穩(wěn)定地；逐步地」。這個詞常與比較級連用。我們來看《經(jīng)濟學人》有關 SAP 的一個例句。

SAP is very late to the cloud, where companies have been progressively moving for the past 20 years, says Liz Herbert of Forrester Research, a consulting firm.

bespoke doses

bespoke 有 spoke，但卻不是「說話」，而是 made specially, according to the needs of an individual customer「訂做的」。但我們可以這樣聯(lián)想：如果不開口對說，對方怎么知道我們想要訂做什么樣的產(chǎn)品？不表達自己的要求，就不叫個性化定制了。dose 是「劑量」，所以 bespoke doses 是「定制劑量」。
